一、 美國FMC無船承運(yùn)人備案
Record of American NVOCC on China’s
　　1、 申請書（格式由代理提供）；
　　Application (Samples to be supplied by the agent)
　　2、 可行性分析報(bào)告（格式由代理提供）；
　　Feasibility analysis report (Samples to be supplied by the agent)
　　3、 企業(yè)商業(yè)登記文件，須由企業(yè)登記機(jī)關(guān)確認(rèn)或證明該復(fù)印件與原件一致的公證文書原件，需大使館認(rèn)證；
　　Business registration document of enterprise, with copies notarized to be fitted with the original and verified by PRC’s embassy; Notarization and verification must be original.
　　4、 提單原格式樣本（背面條款要清楚，要使用CARRIER，不能用FREIGHT FORWARDER）
　　Original sample of bill of lading (items must be clear on the reverse side; CARRIER must be used, but not FREIGHT FORWARDER)
　　5、 聯(lián)絡(luò)機(jī)構(gòu)說明書，載明聯(lián)絡(luò)機(jī)構(gòu)名稱、住所、聯(lián)系方式及聯(lián)系人、簡介（中國簽章）；
　　Description of the contacted organization, with the organization’s name, office, contact ways, contact person and its general introduction (to be chopped with the Chinese office’s common seal)

　　Contract between the client and the contacted organization (Samples to be supplied by the agent)
　　8、 聯(lián)絡(luò)機(jī)構(gòu)工商登記檔復(fù)印件（中國簽章）；
　　Business license of the contact organization (copy to be chopped by the Chinese office’s)
　　9、 法人代表、投資方、2名海運(yùn)部負(fù)責(zé)人簡歷；
　　Resume of the legal representative, the investor and 2 shipping operators;
　　10、 提供FMC下已交存12.5萬美元保證金的證明及美國NVOCC證書復(fù)印件。
　　Proof of depositing 96,000 USD into FMC and copy of American NVOCC certificate.

　　一、申請NVOCC材料清單：

　　（3）企業(yè)商業(yè)登記文件復(fù)印件（境內(nèi)企業(yè)提供營業(yè)執(zhí)照）
　　（4）提單格式樣本正本原件（1式3份不蓋章）
　　（5）若是外資企業(yè)，還需提供外商投資批準(zhǔn)證書復(fù)印件。
　　（6）章程復(fù)印件（大連企業(yè)需要）
　　（7）受理表（廣東企業(yè)提供格式，遼寧企業(yè)現(xiàn)場填寫）
